Money Follows the Person: A 2011 Survey of Transitions, Services, and Costs

Summary

A total of 44 states have received federal grant money to transition Medicaid beneficiaries out of institutions & back to their homes or community through the Money Follows the Person (MFP) demonstration. Results from an annual survey indicate that 17,000 individuals have transitioned back into the community, with 46% of the transitions taking place in Ohio, Texas, & Washington. Affordable housing & workforce capacity continue to be challenges for the program.
