Quality’s New Frontier: Reducing Hospitalizations and Improving Transitions in Long-Term Care

Summary

Review efforts to measure and reduce potentially avoidable hospitalization and improve care transitions for individuals who use long-term care. Three areas in which progress is needed are described. The authors conclude with questions deserving more research to fill gaps in knowledge on how best to improve care quality.
