The Alabama Department of Rehabilitation Services (ADRS) Medicaid Infrastructure Grant developed this Logical Model to illustrate the collaboration project in the demonstration sites. The Model charts the phases of resources, activities, outputs, outcomes (short-term, intermediate, and long-term).
