This chart compares all the various tools used in the MA long term care system to the Minimum Data Set – Home Care (MDS-HC). This format made it easy to see where there where gaps and redundancies in data elements. Sections include personal information (identification, referral, assessment) patterns (cognitive, community, vision, mood and behavior, social functioning), and health (support services, IADL, continence, disease, nutrition, medication) and other service utilization questions.
